For a submarine captain, it means sticking their head out into the fresh air. Ditto for a survivalist living in their underground bunker...though they probably won't try it without a radiation suit and a gas mask.
In oil drilling, the term "topside" refers to the parts of the offshore rig that exist above water. So...you've got the series of tubes and equipment under the waves, drilling down into the ocean floor, getting the oil out. Then you've got all the stuff above the surface. The helipad. The basketball court that hasn't been used since the ball went overboard during the first game. That kind of thing.
